  • Certificate of Good Moral Character
  • PSA Birth Certificate (Photocopy)

  • Long Brown Envelope

02 Transferees

  • Honorable Dismissal / Transfer Credential
  • Transcript of Records (Informative Copy)
  • Certificate of Good Moral Character
  • PSA Birth Certificate (Photocopy)
  • 2 pcs 2x2 ID Picture

Enrollment Process

Submit Requirements

Visit the Registrar's Office and submit all necessary documents for evaluation.

Assessment & Interview

Proceed to the Guidance Office for interview and Accounting Office for fee assessment.

Payment & Registration

Pay the enrollment fee at the Cashier and receive your Certificate of Registration.
